the Critical Care Units of acute care hospitals.

One of our Case Managers will respond within 24 hours and begin the evaluation and intake process. The
clinical evaluation is on-site, when appropriate.
Having the following information available for the Sparrow Specialty Case Manager will assist in the efficient
processing of your referral:
- Patient medical information (condition, medical course) and plan of care
- Patient demographics and payor information

most insurance plans and work closely with the family, physician, referring hospital and payor to ensure a

The discharge process begins before the patient is admitted to Sparrow Specialty Hospital. It is our goal to
help each patient achieve the highest level of function, and discharge as timely as possible. Under the
direction of a physician, our case management team works with our interdisciplinary team, the patient and their
family toward a successful and timely discharge.
